About Colonial Care Center
Colonial Care Center provides skilled nursing, rehabilitation, behavioral, and sub-acute services in a clinical setting focused on safety, continuity of care, and resident outcomes. Our interdisciplinary team supports individuals requiring short-term recovery or long-term care, with admissions available seven days a week. Families are encouraged to tour our facility to learn more about our care approach.
Our Mission
We believe in patient-centered skilled nursing and rehabilitation care, delivered by an interdisciplinary clinical team. Our approach focuses on individualized treatment planning, safety, and consistency to support each resident’s medical and functional needs. Families and care partners are encouraged to participate throughout the care process.
Our Services
Skilled Nursing
A licensed staff offers 24-hour skilled nursing care. Compassionate caregivers provide close medical supervision, which may include assistance with dressing, bathing, walking, eating, and the administration of medication.
Rehabilitation Therapies
A skilled team of physical, speech and occupational therapists evaluates the needs of each resident and, in conjunction with the resident’s family and attending Physician, designs an outcome-oriented treatment program.
Sub-Acute Unit
Our 36 bed Sub-acute unit offers care to the patients who no longer require hospital acute care but require more intensive licensed skilled nursing care. Our unit accepts tracheostomy, ventilator dependent residents, acute stroke, and patients for rehabilitation.
